Facility Evaluation Report

In conjunction to complaint control #31-AS-20250916083845 and an Annual Required visit, Licensing Program Analysts (LPAs) Angela Panushkina and Michael Cava conducted a Case Management visit to the facility to follow up on a Plan of Correction (POC) that was issued on 09/17/25, for the above complaint control number. On that date, due to an inadequate food supply, the licensee was issued a citation for section 87555(b)(26), General Food Services Requirements. Licensee was given until 09/23/25, to clear this deficiency. Today's visit was to insure deficiency is corrected. Based on LPAs inspection of the facility food service on this date, correction is not met. Licensee will be assessed an immediate civil penalty of $700, backdated from 09/18/25 to 09/24/25, for failure to correct this citation, see LIC 421FC, dated 09/24/25. The licensee will be issued another citation for section 87555(b)(26) with an immediate civil penalty of $250 for repeat violation. See 809D.
