entry to come back, both staff and the administrator stated no, and repeated R1 checked out of the facility voluntarily after a day.
In addition to interviewing the administrator and staff, interviews with five (5) of five residents made. These residents stated they do not know who R1 is.
A review of the facility roster reveal no entry under R1's name.
Based on the information obtained, there wasn't enough evidence to prove that the licensee abandoned R1 at the hospital. Therefore, the allegation is deemed Unsubstantiated at this time. Administrator advised and a copy of this report issued.
