administrator, R1's family wanted to move R1 to another facility. Also, according to the administrator, at admission, R1 had no wounds. As for pneumonia, although R1 may have a cough, R1 made no complaints of heavy coughing, fever or having trouble breathing. If that were the case, administrator and staff would have pursued immediate medical attention. Moreover, because R1 was at the facility for less than twenty four hours, administrator was unable to have R1's licensing forms completed. R1's family moved R1 out of the facility abruptly. Based on the information obtained, it could not be proven that staff neglect resulted in R1 to be hospitalized. Therefore, the allegation is deemed Unsubstantiated at this time.
Staff did not keep the facility free from bed bugs:
In regards to the allegation, it was reported that at admission to the hospital, R1 had bed bugs. Interviews with administrator and staff reveal that R1 was only in the facility overnight, and not more than twenty four hours. R1 was admitted into facility the evening of March 2, 2025, and left with family the morning of March 3, 2025. According to the administrator, R1's family wanted to move R1 to another facility. According to both staff and residents, facility does not have any bed bugs. Interview with five (5) of five residents deny facility having any bed bugs, or any other issues with insects. LPA conducted a tour of the physical plant, which includes inspecting three (3) bedrooms, two (2) bathrooms, and common areas. LPA did not observe any insects/bed bugs during the day of the investigation. Based on the information obtained, it could not be proven that staff did not keep the facility free from bed bugs. Therefore, the allegation is deemed Unsubstantiated at this time.
