Facility Evaluation Report
Licensing Program Analyst (LPA) Michael Cava conducted an Annual Required visit and inspection of the facility. LPA met with the administrator, Lucy Perera, and explained the reason for the visit.
At approximately 12:00pm, LPA took a tour of the physical plant. The facility is a one story building. The smoke alarms and carbon monoxide are dual and interconnected. There are three fire extinguishers. One is located in the kitchen, one in the garage and the other is the hallway. The charge date is November 18, 2025.
Kitchen: The kitchen appliances and fixtures were functional. LPA found a sufficient amount of perishable and non-perishable food properly stored. Knives are stored in the medication closet. Cleaning supplies and laundry detergent are stored in a locked closet by the entrance.
Bedrooms: There are five (5) bedrooms. Four (4) are private and designated for residents' use. Per STD 850, Rooms #1 and #2 are non-ambulatory. Rooms #4 and #5 are for ambulatory clients only. Resident rooms are observed to be properly furnished with appropriate beddings and linens with sufficient lighting and closet space. Room #3 is staff office/workstation. Staff and resident files are kept in a locked cabinet at this location..
Bathrooms: The facility has two (2) bathrooms. One bathroom is in the hallway, and the other bathroom is located at the staff office, in room #3. Both bathrooms were observed to have the proper fixtures, grab bars and non-skid mats. The hot water delivered in the bathrooms were measured between 107 to 109 degrees.
