Facility Evaluation Report

Licensing Program Analyst (LPA), Patrick Shanahan, arrived at the facility and was greeted by facility staff. The LPA's temperature was taken and covid-19 questions were asked before the LPA was allowed entry. The administrator was called and arrived a short while later.

LPA was able to briefly tour the physical plant of the home and no health and safety concerns were observed. All residents have private rooms and they all appeared clean and sanitary.

The infection control portion of the annual CARE tool was completed and no deficiencies were observed
