The inspector’s account

In conjunction with complaint control #31-AS-20260407091355, Licensing Program Analyst (LPA) Michael Cava conducted an Annual Required visit and inspection of the facility. LPA met with the administrator, Myline Olivas, and explained the reason for the visit.

At approximately 10:30am, with the assistance of the administrator, LPA took a tour of the physical plant. Required postings were observed in the entry area. The smoke alarms are hardwired and interconnected. There are carbon monoxide detectors that functions properly. The fire extinguisher is located in the kitchen. The charge date is 08/11/25.

Kitchen: The kitchen appliances and fixtures were functional. LPA found a sufficient amount of perishable and non-perishable food properly stored. Knives were stored in a locked drawer in the kitchen. No cleaning supplies observed accessible and out in the open. A complete first aid kit is maintained in the kitchen.

Bedrooms: There are six (6) bedrooms. Five (5) bedrooms are designated for the residents' use. One (1) bedroom is designated for staff. Of the five bedrooms designated for resident use, four (4) are private and one (1) is shared. Per STD 850, bedroom #5 has the fire clearance to retain one bedridden client. LPA observed that all resident rooms were properly furnished with appropriate beddings and linens with sufficient closet space and lighting.

Bathrooms: There are two (2) bathrooms designated for residents' use. Both bathrooms were properly supplied and had functional fixtures, grab bars and non-skid mats. Hot water temperature was measured between 107 to 108 degrees Fahrenheit. No toxins or cleaning supplies present in each bathroom.
