Facility Evaluation Report
Licensing Program Analyst (LPA) Michael Cava conducted an Annual Required visit and inspection of the facility. LPA met with the administrator, Albert Salunga, and explained the reason for the visit.
At approximately 9:00am, with the assistance of staff, LPA took a tour of the physical plant. Required postings were observed in the entry area. The smoke alarms are hardwired and interconnected. There are carbon monoxide detectors that functions properly. The fire extinguisher is located in the kitchen. The charge date is 08/09/24.
Kitchen: The kitchen appliances and fixtures were functional. LPA found a sufficient amount of perishable and non-perishable food at the facility; properly stored. Knives were stored in a locked drawer in the kitchen. Properly labeled medications were locked in one of the kitchen cabinets. A complete first aid kit is maintained in the kitchen.
Bedrooms: There are six (6) bedrooms. Five (5) bedrooms are designated for the residents' use. One (1) bedroom is designated for staff. Of the five bedrooms designated for resident use, four (4) are private and one (1) is shared. LPA observed that all resident rooms were properly furnished with appropriate beddings and linens with sufficient lighting.
Bathrooms: There are two (2) bathrooms designated for residents' use. Both bathrooms were properly supplied and had functional fixtures. Hot water temperature was measured between 108 to 110 degrees Fahrenheit. No cleaning supplies observed in the bathrooms during the day's visit.
