Complaint Investigation Report
On 9/24/21, a subsequent visit was made and residents were observed eating pancakes with strawberries and bananas on top, bacon, coffee, orange juice and water. Based on the information obtained, the allegation is unsubstantiated.
Allegation: Facility does not have snacks available between meals for residents. Residents interviewed did not corroborate the allegation. Residents indicated they get cookies and crackers for snacks. Staff interviewed indicated residents receive cookies, crackers, and yogurt as snacks. These items were observed in the facility during the initial and subsequent visit. Based on the information obtained, the allegation is unsubstantiated.
Allegation: Residents are not accorded dignity. Residents interviewed did not corroborate the allegation. Residents reported having no issues with the staff. A resident's family member was interviewed and did not report any issues. Staff interviewed did not have any information regarding this allegation. Based on the information obtained, the allegation is unsubstantiated.
Allegation: Facility did not take appropriate measures to safeguard a resident's personal property. Allegedly residents' clothing has gone missing. Residents and resident family member interviewed did not corroborate the allegation. No one reported missing any items. Staff interviewed did not have any information and indicated there were no issues with personal belongings. Based on the information obtained, the allegation is unsubstantiated.
Allegation: Facility did not provide blankets to a resident when needed. Allegedly residents are cold and do not have enough blankets. Residents interviewed did not corroborate the allegation. Bedrooms and common areas were toured. Bedrooms had the required comforters and blankets. Two blankets were observed in the living room. Based on the information obtained, the allegation is unsubstantiated.
Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are unsubstantiated.
