Facility Evaluation Report

Licensing Program Analyst (LPA) Blanca Gonzalez conducted a Case Management- Deficiencies visit in conjunction with complaint control #28-AS-20260114123554. The purpose of this visit is to issue deficiency that is not part of the complaint allegations.

LPA requested SIRs regarding previous falls for resident #1 (R1) and Hospice Care Plan that specifies the need for full bed rails for R1. Records were not available to review at the time of the visit. Deficiency cited.

Per California Code of Regulations, Title 22, and California Health and Safety Code, the deficiencies observed during the visit are documented on the LIC809D.
