The inspector’s account

The investigation revealed the following: Regarding the allegation “Staff left client unsupervised” it was reported that on March 19, 2026, C1 was left in the van unattended for about three minutes from 10am to 10:03am. Due to cognitive abilities, clients 1-5 were not able to answer LPA's questions. LPA interviewed three (3) staff. S2 showed LPA CCTV video footage for March 19, 2026, from 9:57am to 10:06am. LPA observed S3 walking three (3) clients out of the van and into the facility at 10:00am. S4 followed with another client, and walked away from the van. S4 was the last staff member to exit the van and left C1 unattended, alone from 10:00am to 10:03am in the van. S3 returned at 10:03am to move the van from the unloading area to park the van. The video shows S3 entering the van, beginning to reverse but did not park and returned to the unloading area. S3 exited the driver’s side, walked around to the passenger side and is seen walking C1 out of the van and into the facility. S3 stated they noticed C1 was still in the van when they returned to the van to park in the designated parking area. A Civil Penalty is accessed in the amount of $500 for Absence of Supervision.

Based on LPA’s observations and interviews conducted the preponderance of evidence standard has been met, therefore the above allegation is found to be substantiated .

An exit interview was conducted, and a copy of this report along with LIC 9099D, LIC421IM and appeal rights were provided Program Director Santos Dominguez.
