Facility Evaluation Report
Licensing Program Analyst (LPA) Blanca Gonzalez conducted an unannounced Case Management Visit-Incident on 08/08/2025, stemming from incident report received on 06/18/2025. LPA was greeted by Field Supervisor Curtis Arias and the purpose of the visit was explained. Program Director, Santos Dominguez arrived shortly after to assist.
During today’s visit, LPA Gonzalez toured the facility with S3, obtained staff and client rosters, obtained copies of documents related to the incident, obtained contact information for Regional Center Service Coordinator and interviewed staff #1-4 (S1-S4).
According to SOC 341 Report of Suspected Dependent Adult/Elder Abuse received on 06/18/2025, staff self-reported that client #1 (C1) expressed a concern to staff #2 (S2) stating that on Wednesday June 11, 2025, while in the vehicle, staff #5 (S5) showed C1 pictures and videos of people having sex.
An internal investigation was conducted and S5 was suspended until the investigation was completed. S5 was allowed to return to work a week later. Contact with C1 and S5 has been minimized. C1 no longer rides in the transport van with S5. Staff in-service training was provided regarding Civility and Zero-Tolerance.
Regional Center QA conducted an investigation. LPA Gonzalez called Regional Center staff but was unable to confirm their findings at this time.
No further action is required at this time. LPA Gonzalez may return to gather additional documents. No deficiencies were cited during this visit.
Exit interview was conducted. A copy of this report was provided to the administrator.
