Facility Evaluation Report
Licensing Program Analyst (LPA) Blanca Gonzalez conducted an unannounced required annual inspection visit. LPA was greeted by Administrator Mary Anne Nacua and the purpose of the visit was explained.
The facility has a fire clearance approved for six (6) non-ambulatory clients. The facility is a single-family home located in a residential area of Glendora. The home consists of a living room, dining area, kitchen, three (3) client bedrooms, one (1) staff room, two (2) bathrooms, attached garage, front and back yards.
LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following:
Living room and dining area were observed clean and free of obstruction with sufficient seating. Living room contained a fireplace that is not functional. Games and activities were observed stored in living room.
Kitchen was observed clean and free of pests. LPA observed sufficient supply of nonperishable food for one week and perishable foods for a minimum of two days. Soaps, detergents, and cleaning solutions were observed to be stored away from food supplies. Sharps were observed secured in a kitchen drawer.
