Facility Evaluation Report

Licensing Program Analyst (LPA) Blanca Gonzalez conducted an unannounced required annual visit. LPA was greeted by staff and the purpose of the visit was explained. Administrator Agnes Aguila arrived shortly to assist with the visit.

The facility is licensed to serve four (4) Developmentally Disabled clients ages 18-59. Two clients may be non-ambulatory. Currently, the facility has four (4) clients serviced by San Gabriel/Pomona Regional Center.

The facility is a single-story home located in a residential neighborhood in Glendora. The home consists of four (4) resident bedrooms, two bathrooms, living room, kitchen, dining area, den/office, attached garage with laundry, front and backyards.

LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following:

Infection Control: Staff are cleaning and disinfecting throughout the day. Facility has sufficient PPE supplies and has an Infection Control Plan maintained at the facility.
