Facility Evaluation Report
Licensing Program Analyst (LPA) Blanca Gonzalez conducted an unannounced required 1 year inspection visit. LPA was greeted by Teresita Sapanghila, DSP, and the reason for the visit was explained. Administrator, Gemma Rodriguez arrived shortly after.
Facility is licensed to serve (6) developmentally disabled adults, ages 18-59. Four can be non-ambulatory. The facility is operating within the scope of the license.
The facility is a single story home located in a residential area of Glendora. The home consists of: living room, dining area, working/office area, kitchen, laundry room, 3 client bedrooms, 2 1/2 bathrooms, an attached garage, front yard and backyard.
LPA toured the facility with administrator and observed the following:
Living room is clean and has sufficient seating for client. There is a covered fireplace, inaccessible to clients. LPA observed games and other activities for clients.
Dining room is clean and has sufficient seating and adequate lighting.
Kitchen was observed clean and free of debris and pests. Sharps and disinfectants were observed locked and inaccessible, under the kitchen sink. Appliances such as stove and microwave were observed clean and operable. Refrigerator was clean and observed to contain sufficient supply of 2 days perishable food. 7 day supply of non-perishable foods was observed in the pantry.
