Facility Evaluation Report

Licensing Program Analyst (LPA) Blanca Gonzalez conducted an unannounced Case Management Visit-Incident on 09/15/2025, stemming from incident report received on 09/05/2025. LPA was greeted by Supervisor Kalia Hauck and the purpose of the visit was explained. Program Director Reyna Zavala was contacted by telephone.

During today’s visit, LPA Gonzalez toured the facility with Supervisor, obtained staff and client files, obtained copies of documents related to the incident, requested an employee phone number list, requested contact information for Regional Center Service Coordinator and interviewed staff #2-4 (S2-S4).

According to Unusual Incident/Injury Report received on 09/05/2025, licensee self-reported that staff #1 (S1) was seen by associates grabbing individual #1 (I1) by the arm and forcing them into a chair. On 09/05/25, Glendora Police Department arrived on sight and took a report from S1. Glendora Police Officer indicated they did not notice any evidence of abuse on I1 and it would be up to the licensee to investigate.

An internal investigation is being conducted and S1 is on administrative leave until the investigation is completed. Regional Center was notified but have not visited the sight regarding this incident. LPA observed I1 and did not observe any noticeable injuries nor did I1 appear in distress. I1 is limited verbally and has limited comprehension, therefore was not interviewed.

LPA Gonzalez may return at a later time to gather more information regarding this visit.

No deficiencies were observed during today’s visit. Exit interview was conducted. A copy of this report was provided to Supervisor Kalia Hauck.
