Facility Evaluation Report
Licensing Program Analysts (LPAs) Elena Mallett & Luis De Leon, and Licensing Program Manager (LPM) Fernando Fierros conducted an unannounced case management visit to obtain status if the facility has a secured perimeter, delayed egress, and video surveillance system in place. LPAs was greeted by staff in charge Lisa Barnhouse and the purpose for today’s visit was explained. Administrator Pamela Ogot joined the visit at 3:42 PM.
LPAs, LPM, and staff in charge Lisa Barnhouse conducted a physical tour of facility. During the visit, LPAs observed a operating surveillance system in the interior and exterior of the facility and a video monitoring station in the nurse station. In addition, LPAs did not observe the facility to have delayed egress doors throughout the facility. The LPAs did not observe the facility to have secured perimeter operable; however, the fire clearance report dated 04/08/2015 was granted for secured perimeters.
