Complaint Investigation Report
In regard to the allegation “Facility staff handled resident in a rough manner”, it is alleged that staff pushed a resident in care. During interviews with staff four (4) out of four (4) staff stated they have never touched a resident. S1 stated that R1 has dementia and sometimes has behavior issues with staff, but that staff does not touch resident. During interviews with residents three (3) out of four (4) residents stated they have never been handled in a rough manner by staff or touched. Three (3) residents stated they feel safe at facility.
Based on interviews conducted and records reviewed, there is insufficient evidence to support the allegations. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are UNSUBSTANTIATED. An exit interview was conducted, and a copy of this report was given to Assistant Administrator Caitlin Hidalgo.
