Facility Evaluation Report

At approximately 10:30 a.m. on 08/05/25 Licensing Program Analyst (LPA) Nicholas Reed conducted an unannounced complaint visit.

Today’s case management visit was conducted to ensure no clients were residing at the facility or receiving care and supervision. LPA was unable to gain entry to the facility due to no staff being present. LPA attempted to contact the administrator and licensee via telephone at 10:45 a.m. and 10:50 a.m. and received no reply. LPA looked through the fornt windows and side gate and observed no signs of inhabitants.

No immediate health or safety hazards were observed during today’s visit.
