Facility Evaluation Report

Licensing Program Analyst (LPA) Angela Panushkina, along with Licensing Program Manager (LPM) Nichelle Gillyard, and Regional Manager (RM) Angela Whittaker, conducted an unannounced Case Management Visit at this facility. RM, LPM, and LPA met with Elba Khachatryan, daughter of the licensee, and Albert Khachatryan - Licensee and explained the reason for the visit. The purpose of the visit is to issue two Temporary Suspension Orders (TSO). The TSO's effective date is the close of business hour today. The TSOs are for Rossmoyne Hills and Silver Light Care located at 8201 Vantage Ave North Hollywood, CA 91605 (#195850239). Elba Khachatryan was also notified that there is a Temporary Manager at the Silver Light Care facility effective today.

During the visit, two (2) residents were relocated from this location to another licensed facility and one (1) moved in with their family/friends.

During today’s visit, “Order to Individual of Immediate Exclusion” was hand delivered to Albert Khachatryan, for this facility for Albert Khachatryan, Edgar Khachatryan and Knarik Panosyan. One "Order to Individual of Immediate Exclusion: was hand delivered to Knarik Panosyan for Silver Light Care (#195850239). It was determined by the Department that a complaint of conduct inimical was substantiated against Albert Khachatryan, Edgar Khachatryan and Knarik Panosyan, thus, necessitating the Order for Immediate Exclusion from both facilities. The Order regarding Individual Exclusion indicates that Albert Khachatryan, Edgar Khachatryan and Knarik Panosyan are not allowed to have contact with clients or to be present at any facility licensed by the California Department of Social Services. Moreover, at the time of this visit, another licensed facility, Silver Light Care (#195850239) had been also served a TSO with the Temporary Manager (TM) in place.
