Home Care Organization Evaluation Report

Home Care Services Bureau Enforcement Analyst (EA) Ryan Chan arrived at the business office of New Horizon Home Care Services Inc on 12/16/25 for a biennial inspection. Upon arrival, EA was greeted by licensee Vagram Atomyan. The proper posting of business hours and license was observed. The proof of insurance records were reviewed. EA was then shown to an area where the review of records could be reviewed. Licensee stated he does not have any clients or home care aides at this time. EA went over home care aide requirements such as training, tb, home care aide registry, and background checks. Upon completion of the file review EA discussed the findings of the inspection with the licensee and informed him that no discrepancies were found.

EA Chan concluded the visit with an exit interview and provided a copy of this report to the licensee.
