Complaint Investigation Report
During interviews with staff, all staff stated that on the evening of 09/13/2023, Staff #3 (S3) fell asleep while on camera duty and Staff #4 (S4) was assisting other residents when the delayed egress alarm sounded. Staff #1 (S1) and Staff #2 (S2) added that police brought R1 back to the facility before S3 and S4 could finish their room-to-room inspection for any wanderers. Record review revealed R1 requires safety measures to address behaviors such as wandering.
Based on interviews, there is enough information to verify the allegation. Therefore, the allegation is SUBSTANTIATED at this time.
Pursuant to Title 22 Division 6 Chapter 8 of the CA Code of Regulations, the following deficiencies were cited (refer to LIC 9099-D):
No health and safety hazards noted during the visit.
Exit interview conducted and a copy of the report was issued.
