Complaint Investigation Report

Regarding the allegation: Staff are not preventing resident from being threatened by another resident while in care. It is being alleged that Resident #2 (R2) threatened Resident #1 (R1). Let it be noted, the threat did not happen at the facility and/or with facility staff but at the job program. During LPA's interview with R1, R1 stated, "that while they were waiting for the job program to let them in, a couple of co-workers saw R2 kissing another co-worker and everyone started laughing." R2 then looked at R1 and stated, "what are you laughing at me, I will shoot you." R1 continued to state to LPA, "we were both suspended from the job and R2 apologized to me but I don't believe it's true." During LPA's interview with R2, R2 stated, "R1 tried to get on my nerves, they lied to everyone, they smirked and made fun of me and we got into an argument outside the job." R2 also stated to LPA, "I don't even have a gun so why would I say that." LPA conducted three (3) staff interviews that work at the facility. During LPA's interview with Staff #1 (S1), S1 confirmed that Regional Center had a meeting with R1, R1's father, R2, R2's father, R1 and R2's boss was present and two (2) other Regional Staff. S1 confirmed that R2 did admit that they threatened R1 and R2 apologized to R1. During LPA's interview with Staff #2, (S2) confirmed that the police did come to the facility to speak to R1 and R2 but R1 was not available at the time to speak to the police but R2 did speak to the police along with R2's father. During LPA's interview with Staff #3 (S3), S3 confirmed that they were not present for the incident since it happened at the job site and also confirmed that R1 and R2 are transported via a third party transportation called Open Door paid by Regional Center and facility staff are not present at the job site. Regional Center and the job site have their own staff." During LPA's interview with R1 and R2's boss from Regional Center, their boss confirmed that both R1 and R2 had a meeting at Regional Center and they were both suspended for their behavior at the job program. In addition, R1 and R2's boss admitted that R2 did admit they threatened R1 and apologized to R1 during the meeting. Therefore, based on the staff and resident interviews conducted and because this incident did not happen at the facility and/or with facility staff, the allegation(s) is UNSUBSTANTIATED at this time.
