Complaint Investigation Report
During interviews with the executive director and five (5) staff members, it was revealed that they are not aware of any foul-smelling odor within the facility. An interview with staff revealed that housekeeping cleans the residents' bedrooms once a week or more when requested. R1 reported being the only person able to smell the alleged odor. LPA inspected R1’s bathroom sink, shower, and toilet, and did not experience any foul smell. LPA interviewed four (4) residents on the floor and six (6) other residents within the facility. The interview revealed that they did not experience or encounter such an odor within the facility.
Based on observations, record reviews, and interviews, there is not enough information to verify the allegation. Therefore, the allegation is UNSUBSTANTIATED at this time.
