The inspector’s account

At 11:50 a.m., Licensing Program Analyst (LPA) Antonia Alvizar-Ettima conducted an unannounced annual inspection at the facility mentioned above. LPA met with Direct Support Professional (DSP’s). DSP Martinez contacted the Administrator via phone and LPA discussed the purpose of this visit. Short time later Administrator arrived and assisted with today’s visit.

At 12:30p.m., LPA and Administrator conducted a physical plant tour inside and out. During the tour, LPA observed that the facility is a home located in a residential community. This is a one-story Adult Residential Facility (ARF) facility licensed for a capacity of six (6) clients. Required postings were observed in the laundry room area. The fire extinguishers are located in the kitchen and hallway with service tag date 04/23/25. Earthquake Emergency drill was last conducted on 05/12/2025. Fire drill was last conducted on 5/18/25.

LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following:

Kitchen: LPA observed kitchen appliances and fixtures were functional. The kitchen has a working gas stove, faucet, freezer, refrigerator, and microwave. LPA found at least two (2) days perishable and seven (7) days non-perishable food at the facility that is properly stored. Frozen foods are wrap, dated, and stored properly as well. Knives were stored in a locked cabinet in the kitchen.

Laundry Room: LPA observed a working washer and dryer next to the kitchen area. Above the washer and dryer are two (2) locked cabinet storing laundry detergents, cleaning agents and other toxins.
