Facility Evaluation Report
On 03/02/26, at 10:50am, Licensing Program Analyst (LPA) Gina Saucedo arrived at the facility to conduct an unannounced, annual visit. Edgardo Jr. Vista, DSP-Direct Support Professional was advised of the visit and toured with LPA.
LPA asked for the census, client, and staff files.
The facility has been licensed as a Adult Residential Facility. All four (4) clients are ambulatory. The physical plant was toured inside and outside at 12:15pm. It is a single, story home.
Kitchen area was sufficiently stocked with seven (7) days of perishable and seven (7) days of non-perishable food. There is one (1) refrigerator in the kitchen area. The cabinets have canned goods. Sharps/Knives are kept secured and locked in one (1) of the cabinets on your right-hand side. The medication is kept in the kitchen area locked and secured in a pantry area. There are toxins that are kept under the sink locked and inaccessible to clients. There is a fire extinguisher fully charged and dated 07/2026.
There is one (1) washer and dryer locked in a room leading towards the backyard. There is another fire extinguisher fully charged and dated 07/2026.
