Facility Evaluation Report

On 05/28/26 Licensing Program Analyst (LPA) Villegas conducted a case management visit to the facility for the purpose to conduct a final walk-through to close the facility and to retrieve the Department of Social Services Community Care Division License. LPA met Administrator Frances Snoddy.

LPA conducted a tour of the entire facility which consist of the following: (6) bedrooms (2.5) bathrooms, living room, kitchen, dining area, shaded patio area on the side of the house, and the garage. LPA observed all rooms to be empty with no clients in care. Prior to closure the facility had 1 clients in care who as of relocated on 03/30/26.

The Administrator Frances Snoddy notified the department on 05/14/26 that she is voluntarily surrendering the facility license due to the landlord not renewing the homes lease. The Administrator is aware she will no longer hold a license to operate from Community Care Licensing Division for this facility #198320007. The Westside Regional Center is aware of the facility closure. The Administrator provided LPA with the facility License as it will no longer be valid.
