Facility Evaluation Report
On 3/3/26, Licensing Program Analyst (LPA) Felisa Shirley conducted a case management visit. The purpose of this visit is to perform an unscheduled compliance inspection. The census is currently, 2.
At 2:39 pm, LPA Felisa Shirley and Beverly Brinson toured the facility inside and out. The facility consists of three (3) bedrooms, in which 2 are occupied and there is one vacancy, two (2) bathrooms, living room, kitchen, dining room and an attached garage. There is an enclosed washer and dryer located in the kitchen. The facility had an ample supply of perishable and nonperishable food items, stocked first aid kit, all medications, toxins and sharp objects were locked and inaccessible to clients. The water temperature delivered at 120 F. The facility maintains a comfortable temperature throughout. The facility has a sufficient linen supply. In the backyard there are no obstructions nor bodies of water. There is a patio table with an umbrella in the backyard.
An exit interview was conducted, and a copy of this report was left with the Administrator, Beverly Brinson whose signature on this form confirms receipt of this document.
