Facility Evaluation Report
On 11/19/25, Licensing Program Analyst (LPA) Felisa Shirley arrived at this facility to conduct a case management visit. The purpose of this visit is to correct the citations issued during the Annual visit conducted on 9/17/25.
Corrected deficiencies are being cited based on observations and records reviewed in accordance with the California Code of Regulations, Title 22, Division 6 chapter 1.
The 809-D’s created during todays visit will supersede the 809-D’s created on 9/17/25.
An exit interview was conducted, Plans of Corrections were discussed and a copy of this report and appeals rights were left with the Administrator, Beverly Brinson whose signature on this form confirms receipt of these documents.
