Facility Evaluation Report
On 7/10/24, Licensing Program Analyst (LPA) Felisa Shirley conducted an unannounced annual required visit with a primary focus on Infection Control measures. LPA met with Nutritionist Services Coordinator, Ericka Yamashiro and explained the purpose of today’s visit. The facility is licensed for sixty (30) ambulatory clients of which twenty-four (24) may be non-ambulatory. There are eight (8) clients in attendance today.
Gardena Senior Citizens Day Care Center is a one-story structure located on the grounds of a church. The facility consists of (1) administrative office, (1) kitchen area, (1) activity room, (2) bathrooms, and (1) storage room. Facility maintains all required posting throughout the facility. LPA checked (4) staff files and (4) client records.
LPA Shirley and Ericka walked through throughout the facility and all common areas such as the activities room, bathrooms and outdoor patio were all observed. The food is delivered here to the day program and LPA observed food in the refrigerator. The sharps are stored in the kitchen locked and inaccessible to clients in care. Bathrooms were found to be within Title 22 regulations and were clean and operational. The water delivered at 118.8 F.
LPA observed that there is ample seating and space for all clients. All walkways were clean, and clear of obstructions and hazards. All areas have ample lighting. There are (5) charged fire extinguishers. Exterior areas are clean and clear of obstructions and hazards and there are no bodies of water present. The first aid kit was fully stocked and there was a first aid manual available. There are no medications being kept at the program.
No deficiencies were cited during this inspection visit.
An exit interview was conducted, and a copy of this report was provided to Ericka Yamashiro.
