Facility Evaluation Report
On 12/6/23 LPA Felisa Shirley conducted an unannounced visit regarding incident reports received 12/5/23. LPA met with Susie Fuentes and explained the purpose of today’s visit and was granted entry.
Incident reports received 12/5/23 regarding excessive drinking by residents. Resident, (R-1) was made to sit outside until sober, and another resident, (R-2) room was checked for alcohol and confined to their room, which are violations of personal rights. Deficiencies cited under California Code of Regulations, Title 22, Division 6, and Chapter 1 & 6 are being cited on the attached LIC 809-D.
An exit interview was conducted and LPA Felisa Shirley printed and left a copy for Susie Fuentes, Administrator.
