The inspector’s account

On 3/3/26 Licensing Program Analyst (LPA) Felisa Shirley conducted an unannounced Annual required visit using the CARE tool. LPA met with Angel Christopher-Evans and explained the purpose of the visit. The program is licensed to serve fifteen (15) ambulatory and twenty (20) non-ambulatory developmentally disabled adults. Today’s census is 26.

The facility consists of a large multi-purpose room, kitchen, two (2) offices, four (4) activity rooms, three (3) restrooms, and outside has shade and a packing lot. All clients bring their own lunch or bring money to purchase lunch in the community. Lunch is available for clients who are not able to bring their lunch. Medications are not stored at this facility.

As a part of today's inspection LPA reviewed (5) client records, (5) staff records, and observed them to be in complete and in order. Facility's licensing fees are current. The facility's last fire drill was conducted on 3/2/26.

LPA Shirley and Angel toured the physical plant inside and out. There were no bodies of water or firearm/ammunition on the premises. All rooms and areas were checked, all furniture was in good condition, adequate lighting provided, storage for staff personal belongings was observed. Walls and floors were in good repair.

LPA observed the facility to be clean and appropriately furnished at the time of visit. Bathrooms were found to be clean and operational, and within Title 22 Regulations. A comfortable temperature was maintained in the facility. Storage area for cleaning agents and toxins were inaccessible to clients. All fire extinguishers were charged and last serviced 7/2025, and the smoke detector/carbon monoxide alarm was operable. The first aid kit along with a manual was available and fully stocked. The water delivered at 120 F. LPA observed all required postings throughout the facility.

An exit interview was conducted and a copy of the report was provided to Program Director, Angel Christopher.
