Facility Evaluation Report

Licensing Program Analyst (LPA) Felisa Shirley made an unannounced Required - 1 Year inspection to the above Adult Day Program (ADP). LPA Shirley met with Ms. Angel Christopher, Program Director, and Ms. Ariel Sanchez, Program Supervisor. LPA Shirley informed staff the purpose of today's visit and was granted entry.

The Adult Day Program is located in a single story commercial building, which consist of a living room, kitchen, 2 restrooms, 4 activity rooms, 2 offices, outside has shade and the parking lot. All clients bring their own lunch or bring money to purchase lunch in the community. If clients forget their lunch the staff has emergency food available.

LPA Shirley and Angel walked throughout the facility and all common areas. All rooms and walkways were clean, and clear of obstructions and hazards. All areas have ample lighting. All rooms and offices have working smoke detectors. There are 2 charged fire extinguishers, 1 located in the kitchen and 1 in the hallway outside of door entry. Both are in compliance. The water delivers at 105 F.

The last Fire/Emergency Drill was conducted on 2/8/2024.

The employee personnel files and client records are all in compliance and locked in a cabinet inaccessible to everyone. The day program currently has zero clients on medications. There are no personal/incidental monies handled by the day program staff. LPA observed a stocked first aid kit with the required manual.
