Facility Evaluation Report
On 9/24/25, Licensing Program Analyst (LPA) Felisa Shirley conducted an unannounced annual required visit. LPA met with Program Director Angel Christopher-Evans and explained the purpose of today’s visit. The facility is licensed for sixty (35) ambulatory clients of which twenty-four (25) may be non-ambulatory. There are Twenty-one (21) clients in attendance today.
Easter Seals Southern California Crenshaw is a one-story structure located on a main street, Crenshaw Blvd. The facility consists of (2) administrative offices, (1) living room (1) kitchen/activity room area, (3) work rooms (3) bathrooms, (1) computer area, (1) refreshing room, 1) nurse’s office and (1) storage room. Facility maintains all required posting throughout the facility. LPA checked (5) staff files and (4) client records. The annual fees are current.
LPA Shirley and Angel walked through throughout the facility and all common areas such as the living room, kitchen/activities room, and bathrooms were all observed. LPA toured the nurse’s office and observed that all medications were safe, locked and inaccessible. The sharps are stored in the kitchen locked and inaccessible to clients in care. Bathrooms were found to be within Title 22 regulations and were clean and operational. The water delivered at 110.5 F. The last fire drill was on 8/11/25.
LPA observed that there is ample seating and space for all clients. The clients bring their own lunches and the facility staff also maintains an emergency food supply for clients. Today the facility staff and clients celebrated taco Wednesday. All walkways were clean, and clear of obstructions and hazards. All areas have ample lighting. There are (3) charged fire extinguishers and all serviced 5/25. Exterior areas are clean and clear of obstructions and hazards and there are no bodies of water present. The first aid kit was fully stocked and there was a first aid manual available. Staff successfully responded to a choking incident involving a client, with all available personnel providing immediate assistance.
No deficiencies were cited during this inspection visit.
