Facility Evaluation Report
On 8/20/25, Licensing Program Analyst (LPA) Felisa Shirley conducted an unannounced annual required visit. LPA met with the Executive Director, Jenna Astleford and explained the purpose of today’s visit. The facility is licensed for seventy-seven (77) clients ages 18-59 of which thirty-nine (39) may be ambulatory and thirty-eight (38) non - ambulatory. There are forty-one (41) clients in attendance today.
Adult Development Center - Gardena is a one-story structure which consist of (1) reception area/lobby, (1) administrative office, (1) kitchen area, (2) classroom/activity room, and (4) bathrooms. Facility maintains all required posting throughout the facility. LPA checked (5) staff files and (5) client records. There are no medications being kept at the program.
LPA Shirley and Jenna walked through throughout the facility and all common areas such as the classrooms/activity rooms, bathrooms and outdoor patio were all observed. The staff use the kitchen to prepare lunch and the clients have access to the microwave. The sharps were locked in a drawer inaccessible to clients in care. Bathrooms were found to be within Title 22 regulations and were clean and operational. The water delivered at 118.9 F.
LPA observed that there is ample seating and space for all clients. All walkways were clean, and clear of obstructions and hazards. All areas have ample lighting. There are seven (7) charged fire extinguishers. Exterior areas are clean and clear of obstructions and hazards and there are no bodies of water present. The first aid kit was fully stocked and there was a first aid manual available. There are no medications being kept at the program.
No deficiencies were cited during this inspection visit.
An exit interview was conducted, and a copy of this report was provided to the Executive Director, Jenna Astleford.
