Licensing Program Analyst (LPA) Tihesha Smith conducted an unannounced case management visit at this facility at 9:45 am. LPA Smith was greeted by staff and disclosed the purpose of the visit.
The purpose of the purpose of today’s visit is to verify excluded employees are no longer working at the facility. LPA Smith conducted a walk through and did not observe staff #1 (S1) or staff #2 in the facility. House manager revealed the staff do not work here.
No health or safety hazards observed during visit.
