The inspector’s account

LPA did a record review today at 10:15 AM and revealed that the admission agreement of the facility includes a copy of the facility rules wherein all clients, upon admission informed and signed the house rules specifically stating that smoking/ vaping inside the facility is strictly prohibited. LPA interview with all five (5) out of six (6) clients between 9:31 AM to 12:30 PM. The interview revealed unanimously stated that staff was not seen smoking/ vaping in the facility. LPA interview with the Chief Operating Officer (COO), Program Director, and four (4) mental health staff. The interview revealed that smoking within the facility is prohibited and staff has not witnessed any staff or client smoking/ vaping in the facility. The alleged staff does not even smoke/vape.

Therefore, based on record reviews, observations, and interviews, the allegation is deemed unsubstantiated .

Allegation #2: Staff do not provide adequate food service

It was alleged that the facility would feed the clients cold food left out for hours. LPA interviews staff and clients from 9:31 to 1:15 PM. The interview revealed that food is being cooked at the facility for lunch and dinner on Mondays, Tuesday, and Fridays. During Wednesday and Thursdays, the facility chef will be cooking at Montare at the Lake (Facility #197610020); which is four (4) minutes away. For the weekends, the facility chef would have food prep for the clients. Continental breakfast is served in the morning, facility would serve cereal, oatmeal, yogurt, bagels, etc. During the visit, LPA observed food to arrive on time just for lunch at 1 PM and LPA observed that the food was properly sealed and hot. LPA also observed an abundant stock of food in the refrigerator, pantry, fruits, and snacks in the facility. Interviews with staff also revealed that food is kept in the wrap and kept in the refrigerator for a few hours after lunch is served. If clients want to eat, food is self-service.

Therefore, based on record reviews, observations, and interviews, the allegation is deemed unsubstantiated at this time.
