The inspector’s account

Licensing Program Analyst (LPA) Daniel Konishi conducted an unannounced Required-1 year visit using the full Care Compliance and Regulatory Enforcement (CARE) Tools. LPA was met by Claudia Renteria, Care Staff and explained the purpose of today's visit. The Administrator, Robert Risinger arrived shortly after and LPA explained the purpose of the visit. The facility is approved for capacity of (6) non ambulatory age range 18 through 59 of which (1) may be bedridden; bedridden to reside in Room #5.

LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following:

Infection Control: Infection control practices and Personal Protective Equipment (PPEs) were maintained. Staff are adhering to infection control requirements. The facility has an Infection Control Plan in place.

Operational Requirements: The Infection Control Plan is in place. A fire clearance is in place. Valid Liability Insurance policy is in place. The last Fire Drill was conducted on 09/03/2025.

Physical Plant & Environment Safety: This facility is a single-story home consists of kitchen, dining room, living room, (4) client bedrooms, (1) office, (4.5) bathrooms, laundry area, indoor/outdoor activity space, attached garage, and a shaded patio with seating. Currently, there is (1) client residing in the home. Client bedrooms were toured. Each bedroom has a smoke detector, bed, linen, dresser, light, chair and sufficient closet space. Client beds and furnishings are in good condition. Carbon monoxide detectors were tested and operable. The laundry area is in the hallway. The fire extinguisher in the dining area is fully charged and purchased on 08/19/2025. Sharps are locked in the kitchen cabinet and inaccessible to clients. Cleaning solutions and disinfectants are locked under the kitchen sink and inaccessible to clients. Extra linens and towels are in the cabinet in the dining area.
