The inspector’s account

Licensing Program Analyst (LPA) Christian Gutierrez conducted the annual inspection using the Compliance and Regulatory Enforcement (CARE) tools. LPA met with DSP Hussein Muhidin and explained the purpose of today’s visit. Administrator Gamal Tawfik was notified by telephone. The facility is licensed for four (4) clients age range 18 through 59, approved for non-ambulatory in rooms 1, 2 and 4 only and are operating within the scope of their license. The facility is vendorized with South Central Los Angeles Regional Center. The facility is a single-story home located in a residential area of Downey with four (4) bedrooms, two (2) full bathrooms, dining room, kitchen, living room, office area, and backyard with a shaded patio area.

LPA toured the facility and observed the following: All client bedrooms have the required furniture and bedding. There is extra clean linen and towels in hallway cabinets. LPA observed bedroom one (B1) to have a door leading outside with no devise to alert staff if he/she were to leave. During record reviews it was discovered that C1, who resides in bedroom #1, has a history of eloping. Smoke detectors/carbon monoxide detectors were observed throughout facility. The facility has one (1) fire extinguisher which is kept in kitchen. LPA observed laundry detergent accessible to clients in laundry area. There is cleaning supplies and toxic substances inaccessible locked in cupboards in kitchen cabinet and laundry room. Freezers are maintained at a temperature of 0-degree F and the refrigerators at a maximum of 40 degrees F. Sufficient supply of 2 days perishable & 7 days non-perishable foods was observed in the kitchen. There are no firearms or weapons stored at the facility. The hot water temperature in the bathrooms were measured between the required range of 105-120 degrees F. The facility does not have a swimming pool or large body of water. There is a shaded seating area for the residents located in backyard. Passageways and exits are free of obstruction

***Due to time constraints, LPA was not able to complete the annual inspection for this facility. LPA will do a continuation of this inspection. ***

Per California Code of Regulations, Title 22, and California Health and Safety Code, the deficiencies observed during today’s visit will be documented on LIC809-D. Exit interview was held and a copy of the report with appeal rights was given.
