Facility Evaluation Report
Licensing Program Analyst (LPA) Blanca Gonzalez conducted an unannounced required annual visit. LPA was greeted by Frederick (Erick) Violago, staff and the purpose of the visit was explained. Jeffren Mungcal, staff and Designated Responsible party in Administrator’s absence, arrived shortly after.
The facility is licensed for four (4) clients age range 18 through 59, approved for non-ambulatory in rooms 1, 2 and 4 only and are operating within the scope of their license. The facility is vendorized with South Central Los Angeles Regional Center.
The facility is a single-story home located in a residential area of Downey with four (4) bedrooms, two (2) full bathrooms, dining room, kitchen, living room, office area, and backyard with a shaded patio area.
LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following:
LPA and licensee toured 4 client bedrooms, 2 bathrooms, a living room, dining room, office area, kitchen, front yard, and backyard. Clients’ bedrooms were checked and were observed to have sufficient lighting and the required furniture with clean linens. Hygiene products are readily available and stored in restroom and a linen closet in the hallway. The hot water temperature was tested in both bathrooms and measured 116.4 degrees F in bathroom #1 and 114.9 degrees F in bathroom #2, which is within the required range of 105-120 degrees.
