The inspector’s account

Licensing Program Analyst (LPA) Christian Gutierrez conducted the annual inspection using the Compliance and Regulatory Enforcement (CARE) tools. LPA met Administrator Shajuana Bradford and explained reason for visit.

The facility is licensed for clients 18-59 years old. The facility has a fire clearance approved for six (6) ambulatory. All clients receive services from South Central Los Angeles Regional Center. The facility is a single-story house located in a residential area in the city of Downey and consists of three (3) client bedrooms, two (2) bathrooms, kitchen, dining room, den/TV room, living room, front yard, backyard, and shed.

LPA toured the facility and observed the following: Each client bedroom has the required furniture and bedding. There is extra clean linen and towels in hallway closet. Smoke detectors and carbon monoxide detectors were observed in each room and throughout the facility and are properly operating. The facility has one (1) fully charged fire extinguisher that is kept in the kitchen. Cleaning supplies and toxic substances are inaccessible to clients in locked cabinet in kitchen. Freezers are maintained at a temperature of 0-degree F and the refrigerators at a maximum of 45 degrees F. A sufficient supply of 2 days perishable food. 7 days of non-perishable foods was observed in the kitchen. There are no firearms or weapons stored at the facility. The hot water temperature in the bathrooms were not measured between the required range of 105-120 degrees F. Bathroom #1 103.9 and Bathroom #2 103.0 TV was issued and water was immediately turned up. The facility does not have a swimming pool or bodies of water on the premises There is a shaded seating area for the residents located in the backyard. Passageways and exits are free of obstruction.

Four (4) staff files were reviewed and included Criminal clearance record, CPR/training, and health screening with TB. Five (5) client files were reviewed and included physicians report, TB clearance, and individual program plan (IPP)report. Last fire/earthquake drill was conducted on 04/01/2026. Infectious control plan was reviewed. Four (4) client medications were reviewed. Medications are centrally stored and locked MAR log is used.

Per California Code of Regulations, Title 22, and California Health and Safety Code, no deficiencies were observed during the visit. Exit interview held and a copy of the report were provided.
