Facility Evaluation Report

Licensing Program Analyst(s) (LPA's) Nune Margaryan and Blanca Gonzalez conducted an unannounced case management visit regarding a client death reported on 04/24/25 to licensing. LPAs met with Administrator Lashon Johnson and explained the reason for the visit.

The department received an incident report on 04/24/25 regarding the death of Client #1 (C1). It was reported that C1 passed away at the Kindred Hospital La Mirada on 04/18/25 approximately at 9:43pm.

Interviewed Administrator stated that on 03/21/25 at 10:15am C1 was founded on the floor in the room. As a precaution staff called 911. Paramedics arrived at 10:30am and transported C1 to Kaiser Permanente Hospital for evaluation. Test results showed that C1 has sacral / coccyx fracture but was stable and did not require surgery. On 04/02/25 C1 was transferred to Kindred Hospital La Mirada to begin rehabilitation. On 04/16/25 C1 was transferred to the ICU at Kindred Hospital because C1 had a heart attack and testing showed that C1 had a complete heart block which will require surgery to place a peacemaker. C1's Family Member (FM) made decision to deny surgery and to stop all life saving measures. C1 passed away on 04/18/25 at the Hospital.
