The inspector’s account

Licensing Program Analyst (LPA) Bennette Pena conducted an unannounced required-1 yr inspection. LPA met with Michael Santos, Program Director and explained the purpose of the visit. The adult day program facility is licensed to serve (60) Developmentally Disabled adults, ages 18 and above, (15) of the clients may be non-ambulatory. LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following:

Infection Control: Staff are adhering to infection control requirements. Hand sanitizers were accessible and observed in high foot traffic areas. Infection control practices and Personal Protective Equipment (PPEs) were maintained. The facility has submitted a COVID-19 Mitigation Plan and the Infection Control Plan.

Physical Plant & Environment Safety: Facility is located in a business complex. It is a single-story building consisting of lobby/reception area, (2) main activity rooms, (3) Administrative offices, (1) conference room, (2) relaxation rooms, (1) changing room, (3) small activity rooms, (4) restrooms (staff & clients), kitchen/break room and (2) storage rooms. Sufficient space to accommodate both indoor and outdoor activities was observed. The interior and exterior physical plant was inspected. Exit doors are free of any obstruction and there are no pools or large bodies of water. There is a drinking fountain in one of the main rooms. The facility does not have a video camera monitor system inside the building. Cleaning supplies and toxic substances are inaccessible to clients. Room for relaxation and rest periods was provided. Bathrooms were clean, toilets and water faucets worked properly. Water temperature readings measured within the required 105 - 120 degrees Fahrenheit. Smoke alarms and carbon monoxide were operable and in compliance. There are no firearms or weapons stored at the facility. Fire & earthquake drills were conducted on 04/21/2026.

Operational Requirements: The facility has the appr opriate fire clearance. Staff do not handle clients cash resources. The fire extinguisher was observed to be fully charged, last serviced on 05/01/2026.
