The inspector’s account

Licensing Program Analyst (LPA) Kimberly Ramirez conducted an unannounced Annual Required Visit on 02/24/2023 at 8:45 am. LPA was met by Staff S1 and explained the purpose of the visit. Facility is licensed to serve developmentally disabled clients 18 and older; 60 ambulatory, of which 15 may be non-ambulatory. Clients at this facility are receiving services from San Gabriel/Pomona Regional Center. LPA requested and obtained a copy of Personnel Report (LIC 500), Resident Roster (LIC 9020), Infection Control Plan-Adult Day Program (LIC 9283) and Mitigation Plan Report (LIC 808).

LPA OBSERVATIONS: Tour began at 9:40 am and was led Staff #1 (S1). The Facility is a single-story building located within in a shopping plaza.

· Lobby: Was clean and well maintained. No hazards were observed.

· Main Floor 1: At 9:42 am LPA observed several clients interacting one on with job coaches. Clients were observed using markers to draw and color on coloring books. LPA several tables and chairs for clients to sit and perform activities. LPA observed a medicine ball rack that contained several different weighted medicine balls in the corner. At 9:48 am, LPA observed digital thermostat to read 68 degrees F. Area was observed to be clean and large enough for clients to roam. No hazards were observed. LPA observed signs promoting hand washing were observed throughout this area. Sketch of facility with emergency exits and location of fire extinguishers was observed.

· Main Floor 2: LPA observed this area to be clean and with plenty of open space for clients to roam. LPA observed several chairs and tables for clients to perform activities. LPA observed a 6ft by 6ft 20 compartment cubby that contained various board games and balls for clients. Several safety mats were observed throughout this area. 24 metal stacked lockers were observed next to cubby. Clients may bring personal items and lock to secure their belongings while attending program. Sketch of facility with emergency exits and location of fire extinguishers was observed. LPA observed signs promoting hand washing throughout this area.
