The inspector’s account

Licensing Program Analyst (LPA) Bennette Pena conducted an unannounced Required- 1 year visit. LPA was met by Efnan Scott, Administrator and explained the purpose of the visit. This facility is licensed as an Adult Residential Care Facility , age range 18 through 59, approved for (4) ambulatory only. Se rvices provided by the San Gabriel/Pomona Regional Center. LPA utilized the Compliance and Regulatory Enforcement (CARE) tools for the visit today and observed the following:

Infection Control: Facility has one (1) person capable and responsible for communicating with emergency personnel in the facility at all times. Contact information of each clients' medical and mental health providers, including emergency agency are current.

Physical Plant/Environment Safety: The facility is a single story home located in a residential neighborhood which consists of (3) client bedrooms, (1) room being used as an office, (2) bathrooms, living room with fireplace, den, dining area, kitchen, laundry area and attached garage. Client bedrooms were toured. Each bedroom has a smoke detector, bed, linen, dresser, light, chair and sufficient closet space. Smoke alarms and carbon monoxide were tested and operable. LPA observed a fire extinguisher next to the kitchen which was last serviced on 08/13/2025. Knives, cleaning solutions, and disinfectants are locked in a cabinet and inaccessible to clients. There are no firearms or weapons stored at the facility. Exit doors are free of any obstruction and there are no pools or large bodies of water. The backyard has a shaded area. The water temperature readings measured within the required 105 - 120 degrees Fahrenheit.

Operational Requirements: The Infection Control Plan has been added to the Plan of Operation. A fire clearance is in place. Fire and earthquake drills are conducted on a quarterly basis. Facility does not handle clients' cash resources. The facility provides a shaded and furnished outdoor activity area. *****REPORT CONTINUED ON LIC809-C*****
