Facility Evaluation Report
Licensing Program Analyst (LPA) Cynthia Chan conducted the unannounced annual inspection using the Compliance and Regulatory Enforcement (CARE) Tools. LPA met with the Administrator, Maria Binotapa, and explained the purpose of the visit. The facility is approved to serve six (6) developmentally disabled adults 18-59 years old, ambulatory only. The clients are receiving services by the San Gabriel/Pomona Regional Center.
The facility is a single story home consisting of (3) shared client bedrooms, (1) staff office, (2) bathrooms, kitchen, dining room, living room, family room, laundry area, backyard with a shaded patio, and an attached garage. The client bedrooms have the required furniture, storage space, and lighting. Smoke and carbon monoxide detectors are operable. Knives, cleaning solutions, and disinfectants are locked. Sufficient food supplies are observed. The hot water temperature was measured within the required 105 - 120 degrees Fahrenheit. The fireplace is not in use and has a cover.
LPA reviewed six (6) client files and their medications. Files are complete with the required documents and medications are being administered as prescribed. There are currently no clients with a restricted health condition. LPA reviewed three (3) personnel files. The administrator's certificate expires on 8/6/26 and the HIV & TB certificates were issued on 9/10/24. Staff have current CPR & First Aid certificates and are receiving annual training. The facility has the updated Emergency Disaster Plan and is being reviewed annually by the administrator. Drills are being conducted at least quarterly with different emergency scenarios.
No deficiencies were observed today. An exit interview was held and a copy of this report was given to the administrator.
