The inspector’s account

On 04/07/2025, Licensing Program Analyst LPA, Yolanda Rosser and Alfonso Iniguez conducted an unannounced Case Management. LPAs met with Nicole Porter/Administrator and explained the purpose of the visit.

On 3/21/25 the regional office received a SOC 341 regarding an incident between (S#1 and C#1). LPA Rosser and Iniguez gathered the following documents and conducted the following interviews:

-Client Roster and Staff Roster, interviews with administrator (A#1), clients (C#1-C#3) and facility staff (S#1).

During this visit, LPAs did not observe any deficiencies therefore no citations were given. A copy of this report was given to Nicole Porter/Administrator.
