Facility Evaluation Report
On 08/10/2026 at 8:33am LPA Zina Brown arrived at the facility to conducted a case management at this facility listed above. LPA informed Frances Snoddy that the purpose of the visit is to conduct a final walk-through and to retrieve the Department of Social Services Community Care License.
At 8:50am, the Department conducted a tour of the entire facility. During the inspection, the Department observed the following: (4) bedrooms, (2) bathrooms, living room, dining room, kitchen, an outdoor laundry area which houses a washer and dryer and patio area on the front porch .
The Department did not observe any food supplies during the inspection. Also the Department did not observe any residents residing at this home.
Braddock Home submitted a closure notification to the Department on 05/29/2026, indicating two (2) residents need a higher level of care. The licensee informed the Department that R1 moved out of the facility on 04/09/2026 and R2 moved out of the 04/15/2026. On 08/10/2026 between the hours of 8:35am - 8:45am, the Department called to confirm the placement for R1 who resides at Chateau of Life as of 04/09/2026 & R2 who resides at Country Cottage 1 as of 04/15/2026.
On 08/10/2026 at 8:53am, Braddock Home has voluntarily surrender the Department of Social Services Community Care License. The original license was given to LPA Brown during the time of the inspection. The licensed facility has been operating since 08/19/2018. Effective as of 08/10/2026 Braddock Home will no longer hold a license to operate from Community Care Licensing.
According to California Code of Regulations, Title 22, Division 6 and Chapter 8, the Department did not observe deficiencies ; therefore no citations are being issued.
