Facility Evaluation Report
On 06/24/2025, Licensing Program Analyst (LPA) Kimberly Ramirez conducted an unannounced Case Management Incident visit regarding the unexpected death of client#1 (C1) on 06/24/2025. LPA Ramirez was greeted by Administrator Linda Williams and explained the purpose of the visit.
On 06/24/2025, LPA Ramirez arrived to conduct an continued annual inspection at the facility and was advised client#1 (C1) passed away at 1am in the facility. According to Administrator Williams, staff#1(S1) was conducting a wellness check on C1 when they realized C1 was unresponsive. 911 was called and S1 began to administer first aid and CPR. Emergency personnel arrived at the facility and pronounced C1 deceased. San Dimas Sheriffs responded and provided Administrator Williams with an incident report. LPA Ramirez toured C1's room and physical plant and did not observe any health and safety concerns.
Interview with Administrator Williams revealed C1 was taken to urgent care on 6/23/25 due to C1 becoming ill with vomiting. C1 was later discharged with new medications. Administrator Williams will provide LPA Ramirez with C1's death certificate once it is filed.
No deficiencies were observed during this visit. Exit interview was conducted. A copy of this report was provided.
