Facility Evaluation Report

Licensing Program Analyst (LPA), Mayra Cota, conducted a Case Management – Deficiencies visit today. LPA met with Eric Howard, Administrator and the reason for the visit was explained.

The citation issued today stems from incidents which were not reported to the department regarding a visitor’s death at the facility which occurred on 3/21/2025. Also, according to the department’s investigation report, the facility did not report a physical assault of a staff member by a client on 12/6/2024, and client being in possession of drugs and drug paraphernalia in the home dating back to 7/8/2021.

Exit interview was conducted with Eric Howard, Administrator and a copy of the report, 809-D and Appeal Rights was provided.
