Facility Evaluation Report
Licensing Program Analyst (LPA) Sanjay Vaid conducted an announced visit to the facility. The facility is designed to serves four (4) developmentally disabled clients 18-59 years old, of which all must be ambulatory only. Currently there are three (3) clients residing.
LPA Vaid met with Licensee Angela Nwaka who assisted with the final inspection of the new room addition to the facility. Fourth room addition.
All documents were provided by the Licensee and approved by the Department. Fire clearance was conducted on 04/08/2026 by LA County Fire Department. Occupancy class is R3.1, fire clearance granted.
The Licensee has added one (1) bedroom to the existing facility floor plan.
Original home was three (3) bedrooms for the residents. Fourth room addition. The new room addition has the required furnishings: bed with linens, night table and lamp, chair, sufficient closet space, pathways are free of debris and obstruction. The fourth room is ready for occupancy.
LPA and Licensee toured the facility and did not observe any health and safety concerns.
Exit interview was conducted and copy of this report was provided to the Licensee, Angele Nwaka.
